Charleston
CHARLESTON — Students who will attend schools in the Charleston school district for the 2008-09 school year can register either Friday or July 28 in the cafeteria at Charleston High School.
The Friday registration session will be from 8 a.m. to 3 p.m. and the July 28 session will be from 2 to 8 p.m. Parking for the sessions will be in the CHS west parking lot and entry to the cafeteria will be through the building’s north doors.
The registration includes students who will attend special education programs in Kansas, Humboldt and Mattoon. The centrally located registration sessions allow families to register students and pay fees at the same time, including if there are children in a family who will be attending different schools. After July 28, students will have to register at the schools they will attend, except special education students who will attend out-of-town facilities, who will register at the district administration building, 410 W. Polk Ave.
Registration fees will be $45 for pre-kindergarten through sixth grade, $60 for middle school and $65 for high school. High school driver’s education fees are $60, and parking stickers for juniors and seniors at Charleston High School are $50.
Insurance coverage for all students is $40, and athletics insurance for CHS and Charleston Middle School athletes is $4 per athlete.
Registration requirements include updated physical and immunization for students entering kindergarten, sixth grade and ninth grade as well as any student new to the district. A school district nurse will be at the registration sessions to check records and medications any student might have to take during the school day.
Also, students enrolling in kindergarten, second grade and sixth grade will need a dental examination from a licensed dentist and a signed proof of examination certificate form from the Illinois Department of Public Health.
Every student’s Social Security number must be listed on their registration cards.
Registration forms, school supply lists and other information are available on the district’s Web site, www.charleston.k12.il.us, and other information is available by calling the district administration building at 639-1000.
Mattoon
The first day of student attendance in the Mattoon school district is Aug. 14 and will last for two hours.
On Aug. 15, students will be dismissed two-and-a-half hours early. The first full day of classes is Monday, Aug. 18.
Mattoon Middle School will host “Discovery Day” from 8:30 to 11:30 a.m. Aug. 8, in which incoming sixth graders have the opportunity to meet their new teachers, find their lockers and tour the school.
Registration for Mattoon school district students in grades K-8 will be held 8 a.m. to 6 p.m. Aug. 4 and 8 a.m. to 1 p.m. Aug. 5. at the students’ respective schools. Registration at Mattoon High School will be held 8 a.m. to 6 p.m. Aug. 4 and 8 a.m. to 4 p.m. Aug. 5.
Children do not need to be present during registration. Parents who are unable to register their children on either of these two days should call their child’s school after Aug. 5 between 8 a.m. and 4 p.m. to make other arrangements.
All district registration forms can be completed and printed prior to registration by accessing the district Web site at www.mattoon.k12.il.us and clicking on the “2008-09 District Registration Forms” link located on the right hand side of the screen. If you do not have access to the Internet, blank district forms will be available to complete at each registration site.
Only children who will be 5 years old on or before Sept. 1 are eligible to enroll in kindergarten. Likewise, only children who will be 6 years old on or before Sept. 1 are eligible to enroll in first grade. Full-day and half-day kindergarten programs are available to all students.
Parents who have pre-registered their child for kindergarten must complete the registration process — pickup parent handbooks and school supply lists; pay fees ($40 for elementary and middle school students); return their child’s school physical and immunization records; apply for free or reduced lunch; meet school officials; etc. — on either of the above dates. All parents should bring their child’s Social Security numbers and the name and telephone number of an adult to contact in case of an emergency during school hours.
All kindergarten, sixth grade, ninth grade, and out-of-district transfer students are required to have had a physical examination within the previous 365 days, and to be properly immunized. Any students in special education programs that will have turned, or will be turning, ages 5, 10 or 15 in 2008 are required to have a physical examination and to be properly immunized.
All kindergarten, second, and sixth grade students must present proof of having been examined by a licensed dentist before May 15 of the current school year. Parents or guardians of children between the ages of 6 months and 6 years must provide a statement from a physician that their child was “risk assessed” or screened for lead poisoning in accordance with state law.
All kindergarten and students that are enrolling in a school district in Illinois for the first time are required to have an eye (vision) exam.
The school nurse will be available during registration to check for compliance. Students who are not in compliance will not be allowed to attend school. However, recent out-of-state transfer students who did not attend Mattoon schools the previous school year may request an extension of this deadline from the school nurse.
In addition, an official copy of the child’s birth certificate (not a hospital certificate) must be presented at the time of registration for a child enrolling for the first time in the district (kindergarten or out-of-district transfer). Students will not be allowed to attend school without first showing proof of a valid physical and immunizations.
Parents will be required to provide evidence that they reside in the Mattoon school district by showing a valid driver’s license or some other proof of residency. All parents will be required to fill out a residency questionnaire. Parents who are divorced and non-parent guardians must provide a copy of their divorce decree or court order granting them legal custody.
Only resident students of the Mattoon school district may attend the Mattoon schools without the payment of tuition. Any person who knowingly or willfully gives any false information to a school district about the residency of a student in order to avoid tuition maybe guilty of a Class C misdemeanor.
Transportation personnel will be available to answer questions about bus routes, schedules, and assignments. Children who reside less than one-and-a-half miles from school are not eligible for bus transportation. Pick-up and drop-off at a childcare provider is available only on established bus routes for eligible riders.
The Mattoon school district allows a limited amount of student transfers between elementary attendance centers. Requests to transfer between elementary schools must be made in writing to the building principal. Approval of such requests will be made solely on the basis of class enrollment that, in some cases, will not be known until after school begins. Transfer application forms will be available at registration.
Students from families that meet certain income requirements are eligible for free or reduced price meals. Information on eligibility guidelines will be available during registration.
